COMMUNITY ACTION NETWORK

It takes a community to care for those who need it the most.

Established in April of 2017, the Community Action Network was developed as a pilot project to better coordinate medical and social services for the patients in East Hawaiʻi who access healthcare services most frequently – patients with complex cases who have many medical and non-medical needs which, if not met, lead to poor health outcomes and higher medical expense.

Over 30 organizations that address social determinants of health needs such as transportation, food insecurity, utilities, housing, and personal safety participate in the network. The network welcomes front-line staff and managers to join in discussions on how to ‘work better together’ with the goal of more effectively caring for East Hawaiʻi community members. An online inventory was created to facilitate referrals among network organizations.

The Community Action Network is led by Nic Los Banos of the Alzheimer’s Association, and Karen Teshima of Blue Zones Project/Sharecare. The following organizations participate:
